What is the difference between an OPG and a CBCT scan?
An OPG (orthopantomogram) is a single two-dimensional panoramic image of both jaws — quick, low dose, and enough for a general survey, third-molar screening or a denture assessment. A CBCT (cone beam computed tomography) captures a three-dimensional volume you can section in any plane, which is what you need for implant site assessment, locating an impacted tooth against the inferior alveolar canal, complex endodontics and pathology. CBCT carries a higher radiation dose, so it is ordered for a specific question rather than as a routine screen.
When should I refer a patient for a CBCT?
When a two-dimensional film cannot answer the question: bone volume and vital-structure proximity before implant placement, an impacted third molar whose roots overlie the canal, suspected vertical root fracture or missed canals in a failing endodontic case, resorption, cysts and other pathology needing extent, TMJ assessment, and orthodontic or orthognathic planning.
